ONLINE FACULTY PERFORMANCE FACULTY EVALUATION FOR TERM 3, SY 2019–2020


February 26, 2020 to March 13, 2020

All Senior High School (SHS), Tertiary Education Department (TED), and Graduate Studies students are reminded to evaluate their professors until March 13 (Friday). The online faculty performance evaluation facility—which provides the university with an objective assessment of its faculty’s planning and preparation, classroom management, instruction and professional responsibilities—can be accessed by logging in to www.dlsau.edu.ph/ebalwasyon-shs/ (for SHS students) and www.dlsau.edu.ph/ebalwasyon/ (for TED, Graduate Studies, and NCP students).

Students may access the facility using their official DLSAU email account at any time, from any desktop or mobile device, as long as it is connected to the internet. Please note that failure to evaluate your professors will result to you being blocked from viewing your grades.

In order to access the facility, new SHS, TED, Graduate Studies, and NCP students are advised to request for their official DLSAU email account from the Information Technology Center by emailing itc.helpdesk@dlsau.edu.ph.

FIRST DLSAU OPEN HOUSE FOR PRESCHOOL AND GRADE 1 LEVELS

WHO-ISSUED HEALTH ADVISORY ON NOVEL CORONA VIRUS


February 10, 2020 until April 25, 2020

In order to help spread public awareness and information on the growing Novel Corona Virus cases (2019-nCoV), World Health Organization (WHO) issued the following health advisory last January 28, 2020:

How can I prevent from getting infected with 2019-nCoV?

Protect yourself from illness with respiratory pathogens by practicing hand and respiratory hygiene and safe food practices:

• Frequently clean hands by using alcohol-based hand rub (for 20–30 seconds) when hands are not visibly soiled or soap and water (40–60 seconds) when hands are soiled;

• When coughing and sneezing cover mouth and nose with flexed elbow or tissue – throw tissue away immediately in a closed bin and wash hands;

• Avoid close contact with anyone who has fever and cough;

• Perform hand hygiene after having contact with respiratory secretions and contaminated objects or materials.

• If you have fever, cough and difficulty breathing seek medical care early and share previous travel history with your health care provider;

• When visiting live markets in areas currently experiencing cases of novel coronavirus, avoid direct unprotected contact with live animals and surfaces in contact with animals;

• The consumption of raw or undercooked animal products should be avoided. Raw meat, milk or animal organs should be handled with care, to avoid cross-contamination with uncooked foods, as per good food safety practices.

• Keep a daily temperature log during duty travel to monitor your health and symptoms during duty travel and for 14 days after returning.

What do I do if I think I have 2019-nCoV?

• If you are experiencing fever, cough and difficulty breathing and have recently travelled to or are a resident in an area where 2019-nCoV is circulating reported, seek medical care immediately if you begin to develop fever or respiratory symptoms such as shortness of breath or cough.

• Before you go to a doctor’s office or emergency room, call ahead and tell them about your recent travel and your symptoms. If advised to seek care, wear a medical mask when you go to doctor’s office/emergency room.

• Do not go to work while sick and minimize contact with others to prevent the spread of infection.

Precautions in health care settings:

• Health care workers (HCWs) caring for patients should apply standard precautions for all patients. See WHO Standard precautions in health care: https://www.who.int/csr/resources/publications/standardprecautions/en/

• HCWs caring for suspected or confirmed coronavirus infection, or handling specimens from them, should implement contact and droplet precautions in addition to standard precautions.

SALARY DEDUCTION OPTION FOR TAAL VOLCANO ERUPTION CASH DONATIONS


February 10, 2020 until April 25, 2020

To support the 700 families who are temporarily evacuated at De La Salle Lipa due to the Taal Volcano eruption last July 12, our Lasallian Mission and Formation Office (LMFO) is hoping to raise P50 000 to help feed and provide for their immediate needs.

Lasallian partners who wish to donate more than P1 000 to Taal Volcano eruption relief efforts may request for salary deduction of up to six (6) pay days starting February 10 until April 25. To confirm your intent to have your donation coursed through salary deduction, please write your name on the form provided by LMFO and submit it to their office on or before February 7 (Friday).

GUIDELINES ON REQUESTS MADE THRU THE ADSERVE HELPDESK


Starting February 6, 2020

In order to facilitate the completion of jobs requested through the Administrative Services (AdServe) HelpDesk, the entire community is advised to take note of the following guidelines:

1. Request should include the following:

a. Description of the job request
b. Location (which must include the building and room number)
c. Name of the requesting person
d. Office of the requesting person
e. Contact number of the requesting person
f. An attachment of a photo, drawing, and other specifications to support the request

NOTE: Requests which lack the abovementioned information will not be processed

2. Requests that do not require budget will be done immediately.
3. Requests that require budget clearance will be estimated in terms of materials and labor cost, and shall be subjected to budget clearance.
4. Requests that require significant funding will be presented to and must be cleared by the Infrastructure Development Committee or the President prior to planning and estimation.

In addition to the guidelines, the requesting party is reminded of the following:

a. The HelpDesk ticket should be CLOSED by the requesting party upon completion of the request. AdServe shall inform the requesting party once the request has been completed.
b. The requesting party must follow up on the request after three (3) working days if there is no feedback received from AdServe.
c. If the request requires immediate action, as in the case of explosion, burning convenient outlets, burning electric fans, and other things that may endanger a person or a property, the requesting party shall call AdServe at any of the following locals 2125, 2143, 1050, 1051 and 1055 prior to posting such request thru the AdServe HelpDesk.
d. Follow-up should not made as a new request. Rather, revisit your request and update your ticket. Duplicate tickets will be closed immediately.
e. For questions and other concerns, please send a private message to the AdServe Messenger account.
