RETURN TO CAMPUS GUIDELINES

To bolster the university’s efforts to keep its campus safe and COVID-19-free, the following health and safety guidelines for its skeleton work force and external clients shall be strictly observed and implemented:


ENTRANCE TO CAMPUS GUIDELINES

STEP 1:
Fill out a health checklist form prior to entry.

STEP 2:
Wear a face mask and face shield upon entry and while inside the campus.

STEP 3:
Submit yourself to the security guard on detail for checking of your body temperature. You will be advised to reschedule your visit to the school if your body temperature Is 37.50c or higher.

STEP 4:
Wash your hands or sanitize them with disinfecting alcohol that is Available at the entrance gate.

STEP 5:
Observe social distancing of six (6) feet at all times. Floor markers are installed to Guide you on safe spots.

COVID-19 HEALTH DECLARATION FORM

Starting January 20, De La Salle Araneta University will be implementing a COVID-19 QR code-based contact tracing tool as part of its health monitoring protocol for its partners, stakeholders, and external guests. Everyone is requested to scan the QR code below which redirects to a health declaration form that one must fill out prior to entry to the campus.


HEALTH MONITORING AND CONTACT TRACING GUIDELINES

STEP 1:
Scan the QR code at the gate using your smartphone’s camera. The QR code is also viewable on the university’s official social media pages and website (www.dlsau.edu.ph/new-normal-protocols/).

STEP 2:
Answer all necessary questions in the COVID-19 Health Declaration Form. Ensure that all information you provide are correct and accurate.

STEP 3:
Click SUBMIT once you’ve filled out the form completely.

STEP 4:
A personal QR code will be generated once you submit the form. Save a copy of this QR code on your smartphone.

STEP 5:
The security personnel will take your body temperature.

STEP 6:
Present your QR code and allow the security personnel on duty to scan the code. Once cleared, you may enter the campus.
